Massachusetts has joined more than a dozen states reporting Cyclospora infections since early May, with health officials working to identify the source of the parasite, which spreads through contaminated food or water and is treatable with antibiotics. The CDC says clusters are being investigated in several states, but no single link has been established.

The parasite causes prolonged diarrhea and other gastrointestinal symptoms such as loss of appetite, bloating, fatigue, low-grade fever, stomach cramps, nausea and vomiting, which can last a month or longer, especially in people with weakened immune systems. Children and older adults may experience more severe illness.

"It’s very different from bacteria or virus," said Lili He, professor and head of the Food Science Department at the University of Massachusetts, Amherst. She noted that standard sanitizing chemicals used to eliminate common bacteria do not necessarily kill Cyclospora, making prevention more challenging.

Dr. Paul Sax, clinical director for the Division of Infectious Diseases at Brigham and Women’s Hospital, said broader testing availability has likely contributed to the apparent rise in cases. "Previously, it would have been much more difficult to make the diagnosis," he explained, adding that the increase is not solely due to a new outbreak.

The FDA is examining white and green onions, cucumbers and cilantro as possible sources, while experts advise thorough hand-washing and washing all produce as basic preventive measures.

"Every infectious disease doctor recommends careful hand-washing and washing all of your produce," said infectious disease specialist Dr. Pax. He cautioned that, although the illness is treatable, patients often feel exhausted for an extended period.
