Measles Exposure Linked to Amtrak Ride Prompts Quarantine of Over 250 in Missouri

A confirmed measles case tied to an Amtrak trip has exposed more than 250 people in Jackson County, Missouri, leading health officials to order quarantine for at least ten unvaccinated individuals and urge increased vaccination.

Jackson County public health officials identified a measles case that originated on an Amtrak train traveling from Sedalia to Chicago on Aug. 27. The infected passenger visited the county’s Lee’s Summit health department building twice, exposing staff and visitors. As a result, the department has traced 251 exposures in the county, including at least ten people who must quarantine for 21 days because they lack documented vaccination.

The investigation also linked the case to a separate measles infection in another Missouri resident who rode the same train, bringing the total number of cases tied to the Amtrak exposure to four. Health authorities in neighboring Johnson County are monitoring potential exposures there, though no new cases have been reported as of Sept. 16.

"If we have pockets of individuals who are unvaccinated or can’t receive vaccines, it gives the virus the opportunity to enter those communities and spread like wildfire," said Ray Dlugolecki, assistant director of Jackson County Public Health.

State data show vaccination rates for the measles-mumps-rubella (MMR) vaccine among kindergarteners have fallen below the 95% threshold needed for herd immunity, with 89.5% in Missouri and 90.3% in Kansas in 2025. The increase in exemption requests has contributed to this decline.

"People have to realize there are constant sparks getting thrown; now that there are more people walking around unprotected, it’s a larger issue again," epidemiologist Nathan Koffarnus said.

The department has deployed a team of six staff members, working over 150 hours to trace contacts, verify immunity, and provide guidance. Officials warned that measles can remain airborne for up to two hours and that unvaccinated individuals face a high risk of infection and serious complications.

The incident underscores the resource intensity of measles responses and the importance of maintaining high vaccination coverage to prevent future outbreaks.
