UPS Retires MD-11 Fleet Following Fatal Kentucky Crash
On November 4, 2025, a UPS McDonnell Douglas MD-11 cargo plane crashed shortly after takeoff from Louisville Muhammad Ali International Airport in Louisville, Kentucky. The crash resulted in 14 fatalities, including all three crew members aboard and 11 people on the ground. The incident caused significant injuries and operational disruptions in the area, prompting a shelter-in-place order within a five-mile radius. The National Transportation Safety Board (NTSB) launched an investigation focusing on a persistent cockpit alarm and the detachment of the left engine during takeoff. Preliminary reports identified fatigue cracking and overstress failures in the aircraft's left engine pylon as potential causes. It was also revealed that Boeing was aware of engine mount failures in MD-11 aircraft as early as 2011. Following the crash, UPS retired its entire fleet of MD-11 cargo planes, and both UPS and FedEx grounded their MD-11 fleets as a precaution.
